Why doesn't my database file grow?

You're probably using Firebird on Windows. Since Firebird locks the database file while there are active attachments (connections) if you look at the file size in Windows Explorer it will seem like the file size doesn't change at all. You should be able to click on file properties from the context menu and see that the file does change.
